The UN warned that the pandemic would disproportionately impact Indigenous people because of underlying health inequities associated with SDOH. This paper can help with “visioning” action for response and recovery. It highlights data, in Hawaii, and 4 other western States, showing that MHPI have high rates of COVID-19. Reliable and timely data are vital (longstanding issues). The higher risk of infection among NHPI is linked to preexisting health inequities. Chronic medical conditions, linked to the SDOH, are high. The paper highlights the economic conditions of the NHPI population, and emerging behavioral health impact of COVID-19. It also mentions the resilience and cultural assets of NHPI Communities, including the values and practices of aloha (compassion), malama (caring) and lokahi (unity). Recommendations: an emergency response plan developed through engagement with NNHPI stakeholders, and a longer term recovery plan to address employment, education, the racial wealth gap, food insecurity, housing, healthcare, criminal justice, and legal issues.

The Native Hawaiian and Pacific Islander community experienced high COVID-19 case rates by April 2020, in addition to chronic diseases and socioeconomic disparities, so NHPI networks were mobilized to address the crisis, and a COVID-19 Response, Recovery and Resilience team was created. The team consists of multiple committees that examine policy; testing, contact tracing, and isolation; communications; social supports and resources; and data and research. This article discusses the development of a cultural framework to guide culturally informed advocacy efforts to ensure an appropriate response and recovery plan for NHPI that extends beyond COVID-19 to addressing longstanding NHPI disparities.

In June 2020, Northwest Arkansas was a COVID-19 hotspot, with serious racial and ethnic disparities. Existing community-engaged partnerships utilized their collaborative capacity to address COVID-19 disparities in the Latinx and Pacific Islander communities. Eighteen key partners held weekly meetings and regular communication and developed four strategies: (1): health education (prevention, testing, quarantine, and follow-up care) and prevention (communication re: practices to reduce risk); (2) testing; (3) a dedicated contact tracing center with bilingual workers; (4) enhanced case management and supported quarantine.

This practice traces a 14-year partnership between health institutions and a rural Native Hawaiian community. The partnership began as a single study to study familial cardiomyopathy, then evolved towards a community-based project to improve overall health and wellbeing. As time went on, other institutions and community leaders became involved. The article focuses on the growth of institutional capacity and community service. Lessons learned can serve to engage communities in COVID-19 prevention and mitigation measures.

This article evaluates a partnership between the University of Maryland, Baltimore – an urban academic treatment center – and multiple rural behavioral health treatment centers to provide medication assisted treatment (MAT) to individuals with opioid use disorder via videoconference. Retention rates and toxicology results for patients who received MAT via videoconference were comparable to those receiving face-to-face treatment. It should be noted that data for this study were collected prior to the onset of the COVID-19 pandemic.

Case Study
This article summarizes the lessons learned from implementing telehealth in rural environments over the past 3 years and how to expand these efforts in the context of COVID-19. The needs of elderly populations in rural environments are also addressed. It provides 5 strategies for implementing telehealth in this context: (1) Preparation and Training; 2) Standardize and Expedite Credentialing; (3) Technology; (4) Broadband; and (5) Feedback and Rapid Improvement.

Summary Report/Recommendations
This article compared community health workers (CHWs) in Indiana and Texas based on the results of two research studies in order to 1) investigate the unique role of CHWs and their Spanish-speaking counterparts, promotores de salud, in rural communities and 2) understand how their advocacy efforts represent a central form of caregiving. An analysis was done of how CHWs connect structurally vulnerable clients in rural areas to resources, health education, and health and social services. CHWs and promotores de salud have acted as first responders to the needs of marginalized rural populations, especially during the COVID-19 pandemic. In addition to health education and translation work, CHWs also play an important role in advocacy, bridging the gap between clients and community leaders. While informing clients of available resources, they empower the clients to advocate for their own needs in different settings. CHWs also have the ability to engage with stakeholders and policy makers in better addressing client needs.

This study evaluated the effectiveness of a telephone-based intervention using a Regional Operational Call Center as a primary method of providing COVID-19 information to rural communities and connecting those at risk with medical appointments. Results indicated that interaction with the call center led to an increased likelihood of scheduling a healthcare appointment and receiving a COVID test.

This article describes a community-based participatory research initiative based on an existing community–academic partnership. Partners included the University of Arkansas, nonprofit organizations serving the Marshallese and Latinx communities, community-based nonprofit organizations, local hospital systems, a federally qualified health center, the Veteran Health Administration, the Arkansas Department of Health, and the Republic of the Marshall Islands consulate. The partnerships were leveraged to develop and implement the Comprehensive Intervention to Reduce COVID-19 Disparities in Marshallese and Latinx Communities in Benton and Washington counties. The comprehensive intervention consisted of 4 components: (1) health education, (2) testing, (3) contact tracing, and (4) care navigation (case management) for supported quarantine. After implementation of the comprehensive intervention, the proportions of new weekly cases among Marshallese and Latinx residents declined and began to align more closely with the proportions of Marshallese and Latinx community members in the 2 counties.
