This episode from the podcast 99 Percent Invisible reflects on how the COVID-19 pandemic exposed the American public to a long-standing problem: the disjointed nature of the U.S. health system. Because state and local health departments largely operate independently and use their own data collection and analysis tools, health systems across the country lack standardized data definitions and systems. These inconsistencies made it nearly impossible to collect and analyze comprehensive, standardized data on COVID-19 cases, deaths, and vaccines administered amid the pandemic. Health experts featured on this episode believe that the pandemic made the need for an overhaul of America’s informatics system very apparent. When rebuilding this system, it’s important to focus on remedying existing inequalities in data collection and classification that in some cases render the health status of certain populations – think Native American communities and other communities of color – completely invisible in the data. By virtue of being small populations, it can be difficult for health departments to collect sufficient and/or statistically significant data on minority communities. Another issue discussed in this episode is the use of broad racial categories like “other,” “multiple races,” or even “Asian American,” which, if not disaggregated, obscures the health status of diverse populations who are grouped under the same category. Without comprehensive and inclusive health data, it’s difficult to identify disparities and implement policies and programming that promote social mobility and health equity.

The article discusses the challenges and frustrations surrounding Long COVID research in the United States. Despite substantial investments and numerous research efforts, progress in understanding and treating Long COVID has been slow and disappointing. Experts and patient advocates have criticized the research community for its focus on disease development rather than direct patient care, duplication of studies, and a lack of clinical trials. The authors propose a new hypothesis that Long COVID may be a form of post-infectious syndrome or chronic fatigue syndrome, suggesting that traditional biomedical research approaches may not yield meaningful results. Instead, they advocate for a shift towards health services research and measures that directly impact patients’ welfare, including prevention, improved prognosis, access to empathetic care, and quality of life issues. They emphasize the need for collaboration between researchers, patients, and advocacy groups to reset the research agenda effectively.

A large school district in Cincinnati partnered with their local health department and children’s hospital to develop mitigation procedures as children returned to school. These mitigation procedures successfully limited school-based spread of COVID-19 by combining best practices in rapid testing, contact tracing, and early isolation. Procedures were regularly revisited and updated in an iterative process to respond to new information and circumstances.

The El Bari Clinic launched a telemedicine initiative during the COVID-19 pandemic to reduce in-person visits and reduce transportation as a barrier to accessing health care. This cases study notes that the clinic offers patient-centered and culturally competent care to under and uninsured community members, which largely includes working-class ethnic minorities.

This paper describes how a local public health agency joined a multi-sector community partnership made up of community-based organizations, clinical providers, and peer navigators and others with lived experience of homelessness. Together, this group created a strategic plan to lessen the impact of COVID-19 on people experiencing homelessness. They prioritized education, increased resource allocation and testing in shelters, and slated housing for those who tested positive or those at increased risk of COVID complications. Most of this work took place in shelters.

This article describes how one county’s health department partnered with emergency services to form a strike team focused on mitigating COVID-19 outbreaks in congregate living facilities. The team helped create more cohesive communication between these facilities and between facilities and the health department. This strike team identified these facilities through partnerships with community organizations. The strike team conducted wellness checks, provided PPE education, and helped facilities secure PPE. In facilities with potential outbreaks, the team provided early testing and quick isolation.

This case study outlines how Sonoma County Department of Health Services created a framework called Health Action, outlining goals for improving the most pressing health disparities in their region. The study also describes how the Department convened a collaborative Health Action Council, comprised of community partners to address the key indicators of health outlined in the framework. They created a report based in data to align the work of the Department and its partners in order to more effectively address health disparities. Some of the most pressing problems identified included disparities around education and the relationship between education and health. Formation of Health Action has resulted in multiple groups using the strategic plan to inform their efforts.

During the pandemic, the Minnesota Department of Health created three new teams focused on an equitable COVID-19 response. These teams included the Cultural, Faith, and Disability Communities Branch; the Tribal COVID-19 Healthcare Team; and the Vaccine Equity Branch. These teams created partnerships with community partners, elevated community voices, set equity goals and metrics, and respected tribal sovereignty in their COVID-19 response. This helped to ensure resources were used equitably and that COVID-19 responses were effective within diverse communities.

This paper discusses how health departments can increase community trust in their work by formally partnering with CHWs and integrating them into the department’s work. In this case study, CHWs were particularly instrumental in educating community members about COVID-19 symptoms, best practices, and more. These CHWs also served as stakeholders in a variety of health department meetings, especially around COVID-19 decision making.
