This study conducted interviews with public health professionals from local and state health departments to understand their experiences in providing comprehensive COVID-19 case investigation and contact tracing with refugee, immigrant, and migrant communities. The research identified six themes related to promising practices, including understanding the community and public health context, cultivating relationships, ensuring linguistic and cultural concordance, communicating intentionally, evolving response strategies, and implementing equity measures. These findings aim to inform proactive, community-engaged solutions for public health organizations working with these communities during public health crises, such as COVID-19, to improve collaborations and staff wellbeing.

Case Study
A large school district in Cincinnati partnered with their local health department and children’s hospital to develop mitigation procedures as children returned to school. These mitigation procedures successfully limited school-based spread of COVID-19 by combining best practices in rapid testing, contact tracing, and early isolation. Procedures were regularly revisited and updated in an iterative process to respond to new information and circumstances.

A retrospective registry-based chart review examined the various demographic and clinical risk factors associated with COVID-19 severity among patients aged 18-29. The study was done within a metropolitan health care system in Houston, TX. In the cohort of 1,853 young adult patients diagnosed with COVID-19 infection at a hospital encounter, including 226 pregnant women, 1,438 (78%) scored 0 on the Charlson Comorbidity Index, and 833 (45%) were obese (≥30 kg/m2). Within 30 days of their diagnostic encounter, 316 (17%) patients were diagnosed with pneumonia, 148 (8%) received other severe disease diagnoses, and 268 (14%) returned to the hospital after being discharged home. In multivariate logistic regression analyses, increasing age, male gender, Hispanic ethnicity, obesity, asthma history, congestive heart failure, cerebrovascular disease, and diabetes were predictive of severe disease diagnoses within 30 days. Non-Hispanic Black race, obesity, asthma history, myocardial infarction history, and household exposure were predictive of 30-day readmission.

Youth nationwide face new intensified stressors like illness, death, social isolation, economic stress, food insecurity, family hardship, and increase domestic violence risk. These stressors are associated with outcomes like depression, behavioral problems, anxiety disorders, and worsened existing mental health conditions. School-based health centers (SBHCs) are a cost-effective health care delivery model that increases mental and behavioral health access and use. SBHC patients live in underserved communities and are at a greater risk for mental health issues. The qualitative data from this study highlight increased demand for mental health services and a lack of resources to meet this demand, resources to ensure universal telehealth technology access are needed to improve health care access for youth, and parental buy-in and support are crucial component of sustained care.

In the United States, mobile health clinics are an important method for delivering high quality care to medically underserved populations. To address declining vaccination coverage among young children in Boston during the pandemic, Mattapan Community Health Center (Mattapan) and Codman Square Health Center (Codman Square) partnered with the Kraft Center for Community Health at Massachusetts General Hospital and the Massachusetts League of Community Health Centers to deploy a pediatric mobile health clinic as an adjunct to their in-person clinical services.

Data from the Adolescent Behaviors and Experiences Survey (AMES) was used by CDC to assess 9th-12th grade student behaviors and experiences during the COVID-19 pandemic. The prevalence of poor mental health and suicidality was high across students of all sex, sexual identity, and racial and ethnic groups; however, poor mental health, persistent feelings of sadness or hopelessness, and suicidal thoughts and behaviors were less prevalent among those who felt close to persons at school and were virtually connected with others during the pandemic. The mental health of youth might be improved via strategies that improve connections with others at home, in the community, and at school.

An early report issued by the CDC identified staff members working in multiple nursing homes as a likely source of spread of COVID-19. The authors performed the first large-scale analysis of nursing home connections via shared staff and contractors. Using a large-scale analysis of smartphone location data, they found that 49 percent of COVID-19 cases among nursing home residents was attributable to staff movement between facilities. Traditional federal regulatory metrics of nursing home quality were unimportant in predicting outbreaks. The results provide evidence for a policy recommendation of compensating nursing home workers to work at only one home and limit cross-traffic across homes.

This case study is on the effectiveness of COVID-19 vaccine distribution via mobile vans to residents/staff of 47,907 long-term care facilities (LTCFs) across the United States that relied on algorithms to optimize vaccine distribution. The authors developed a modeling framework for vaccine distribution to high-risk populations in a supply-constrained environment. The framework decomposed this challenge as two separate problems: an assignment problem, where they optimally mapped each LTCF to select CVS stores responsible for vaccines; and a scheduling problem, where they developed an algorithm to assign available resources efficiently. The learning and this framework may be of use to other organizations, including communities where mobile clinics can be established to efficiently distribute vaccines and other healthcare resources in a variety of scenarios.
