This article discusses the Foundation for Opioid Response Efforts (FORE) distribution of $1.3 million in grants to organizations studying the potential permanent adoption of COVID-19 care access measures to promote health equity in opioid use disorder treatment. The grants aim to investigate the impact of temporary policies, such as virtual care, on access to care and patient outcomes. The studies will explore how relaxed prescribing policies for buprenorphine, telehealth strategies, and other COVID-19 measures have affected patient access and outcomes, particularly among underserved populations. The research will inform evidence-based policies to sustain access to better care beyond the pandemic and address the ongoing opioid crisis.

Article outlines how an opioid treatment program in the Bronx, NY adjusted their clinical practice with COVID-19 and their recommendations for treatment programs going forward. Balancing Covid-19 safety and the need to safely administer medications for opioid use disorder, this program ultimately made changes that allowed for more patient-driven treatment plans that enable fewer in person clinic visits. The article advocates for patient-centered care that encourages people to safely self-manage medication, focusing on patient-centered measures in clinical decision-making, and making appropriate changes to payment/insurance models that allow for such patient-centered care.

This study describes harm reduction and health services provided by the U.S. syringe services programs (SSPs) in 2019 and changes in provision of those services in 2020. Many SSPs that offered health services in 2019 and remained operational in 2020 increased telehealth provision of mental health and primary care services, increased MOUD provision, and expanded harm reduction services, but most SSPs reduced or stopped on-site HIV and HCV testing. With the record high opioid overdose fatality rates in 2020, sustaining the trajectory of SSP growth and innovation leading up to and through 2020 requires ensuring adequate and flexible funding to support the life-saving work of SSPs in preventing future overdose deaths and outbreaks of HIV and HCV.

The UN warned that the pandemic would disproportionately impact Indigenous people because of underlying health inequities associated with SDOH. This paper can help with “visioning” action for response and recovery. It highlights data, in Hawaii, and 4 other western States, showing that MHPI have high rates of COVID-19. Reliable and timely data are vital (longstanding issues). The higher risk of infection among NHPI is linked to preexisting health inequities. Chronic medical conditions, linked to the SDOH, are high. The paper highlights the economic conditions of the NHPI population, and emerging behavioral health impact of COVID-19. It also mentions the resilience and cultural assets of NHPI Communities, including the values and practices of aloha (compassion), malama (caring) and lokahi (unity). Recommendations: an emergency response plan developed through engagement with NNHPI stakeholders, and a longer term recovery plan to address employment, education, the racial wealth gap, food insecurity, housing, healthcare, criminal justice, and legal issues.

The Native Hawaiian and Pacific Islander community experienced high COVID-19 case rates by April 2020, in addition to chronic diseases and socioeconomic disparities, so NHPI networks were mobilized to address the crisis, and a COVID-19 Response, Recovery and Resilience team was created. The team consists of multiple committees that examine policy; testing, contact tracing, and isolation; communications; social supports and resources; and data and research. This article discusses the development of a cultural framework to guide culturally informed advocacy efforts to ensure an appropriate response and recovery plan for NHPI that extends beyond COVID-19 to addressing longstanding NHPI disparities.

In June 2020, Northwest Arkansas was a COVID-19 hotspot, with serious racial and ethnic disparities. Existing community-engaged partnerships utilized their collaborative capacity to address COVID-19 disparities in the Latinx and Pacific Islander communities. Eighteen key partners held weekly meetings and regular communication and developed four strategies: (1): health education (prevention, testing, quarantine, and follow-up care) and prevention (communication re: practices to reduce risk); (2) testing; (3) a dedicated contact tracing center with bilingual workers; (4) enhanced case management and supported quarantine.

Case Study
This article summarizes COVID-19 harm reduction approaches taken by a family medicine/OBGYN residency program serving rural, western North Carolina for patients with an opioid use disorder. To meet patient needs, low acuity patients were offered telehealth options and digital prescriptions for buprenorphine/naloxone. For higher acuity patients, in=person visits were maintained and the risk/benefits were weighed on a continuous basis.

This resource makes recommendations related to establishing crisis stabilization systems for persons living with mental illness. This article describes how pandemic-related changes and funding increases can be leveraged to strengthen statewide crisis stabilization systems for individuals living with mental illness and substance use disorder, including recommendations for specific stakeholders to engage and action steps. The practice strategy this article is focused on is policy and organizational change to improve public health infrastructure.

This resources summarizes the impact of the COVID-19 pandemic on substance use and the experiences of people who used drugs during the COVID-19 pandemic. Highlights include: the pandemic increased the risk of overdose for people who use drugs, and in response people who use drugs reported using more harm reduction strategies. The pandemic also motivated change in wanting to quit drug use for study participants, but also highlighted the social determinants of health as barriers to sustainable recovery.

This practice traces a 14-year partnership between health institutions and a rural Native Hawaiian community. The partnership began as a single study to study familial cardiomyopathy, then evolved towards a community-based project to improve overall health and wellbeing. As time went on, other institutions and community leaders became involved. The article focuses on the growth of institutional capacity and community service. Lessons learned can serve to engage communities in COVID-19 prevention and mitigation measures.
