The article discusses the challenges and frustrations surrounding Long COVID research in the United States. Despite substantial investments and numerous research efforts, progress in understanding and treating Long COVID has been slow and disappointing. Experts and patient advocates have criticized the research community for its focus on disease development rather than direct patient care, duplication of studies, and a lack of clinical trials. The authors propose a new hypothesis that Long COVID may be a form of post-infectious syndrome or chronic fatigue syndrome, suggesting that traditional biomedical research approaches may not yield meaningful results. Instead, they advocate for a shift towards health services research and measures that directly impact patients’ welfare, including prevention, improved prognosis, access to empathetic care, and quality of life issues. They emphasize the need for collaboration between researchers, patients, and advocacy groups to reset the research agenda effectively.

White Paper/Brief
The article highlights the lessons learned from the COVID-19 pandemic and their influence on preparations for future public health emergencies. While the federal Public Health Emergency for COVID-19 is set to end in May 2023, the FDA emphasizes the continued importance of COVID-19 vaccines and treatments. Addressing misinformation and strengthening supply chain resilience are key priorities, and the FDA is developing more efficient inspection planning systems. Modernizing FDA authorities and maintaining a commitment to planning for the future are also emphasized to enhance national security and public health preparedness.

White Paper/Brief
Using a grant from the National Science Foundation, Predictive Intelligence for Pandemic Prevention (PIPP) developed tools and systems to respond effectively to infectious disease pandemics. Recommendations from this work include avoiding reinvention of the wheel by centralizing and standardizing approaches to pandemic response planning, collaboration and information sharing among public health stakeholders, investing in high-performance computing and data management systems for public health, creating a national center for pandemic response, developing playbooks for fast-moving threats, and identifying sustainable and scalable solutions.

Summary Report/Recommendations
This commentary discusses the importance of inclusive health systems strengthening in promoting equity during health emergencies, using the example of disability inclusion in the COVID-19 pandemic. It highlights the increased risks faced by persons with disabilities during the pandemic and the barriers they encounter in accessing healthcare. It emphasizes the need for accessible and comprehensive health emergency preparedness and response plans that consider the needs of persons with disabilities. It also explores strategies for disability inclusion in COVID-19 responses, such as producing accessible public health information and involving organizations of persons with disabilities in decision-making processes. Opportunities to strengthen inclusion and equity in COVID-19 responses include equitable access to telehealth and inclusive decision-making processes.

Peer Review Study
Mask mandates in states beginning in 2020 had a significant impact on COVID-19-related hospitalizations. This study recommends revisiting mask mandates as new variants of COVID-19 spread and pressures return to the healthcare system.

Systematic Review/Meta-Analysis
This analysis explores regional variation in COVID-19 death rates across America. It unpacks the “two Americas” narrative of some parts of the country having high demand for vaccines and others having widespread vaccine hesitancy, and shows that a divide between rates of exceeds death existed prior to the introduction in vaccines. This difference is likely due to a difference in mask use, social distancing, and other behaviors. Results underscore the importance of excess mortality measures such as wastewater testing to get a more accurate picture.

Summary Report/Recommendations
The Massachusetts Executive Office of Elder Affairs, the Massachusetts Healthy Aging Collaborative, and Tufts Health Plan Foundation partnered with FSG to conduct interviews and secondary research of COVID-19 lessons and recommendations framed for three key audiences: organizations, partnerships and collaborations, and funders and policymakers. This research was conducted in six communities in Massachusetts, and identifies key themes and strategies to lift up based on real-world activities implemented during the pandemic. Findings emphasize the disproportionate impact of COVID-19 on older adults across a broad range of social, economic, and health factors, and effective strategies for supporting older adults are outlined such as direct communication and flexible services. Gaps are also identified across each of the findings, indicating where there is room for improvement and community strengthening. Lessons and recommendations are also clearly outlined by audience, with clear suggestions for implementing effective strategies to support older populations in various community settings.
