A large school district in Cincinnati partnered with their local health department and children’s hospital to develop mitigation procedures as children returned to school. These mitigation procedures successfully limited school-based spread of COVID-19 by combining best practices in rapid testing, contact tracing, and early isolation. This case study outlines how Sonoma County Department of Health Services created a framework called Health Action, outlining goals for improving the most pressing health disparities in their region. The study also describes how the Department convened a collaborative Health Action Council, comprised of community partners to address the key indicators of health outlined in the framework. They created a report based in data to align the work of the Department and its partners in order to more effectively address health disparities. Some of the most pressing problems identified included disparities around education and the relationship between education and health. This article describes the impact of COVID-19-specific adaptations at two sites delivering a federally funded substance use treatment program for people living with or at-risk of HIV in Massachusetts. The success of the adaptations varied based on the site. For example, the transition to telemedicine was successful at the STI clinic, but not the community-based organization. However, the article does identify some adaptations that were successful in each setting, and the authors report process outcomes for these adaptations. The practice strategy this article is focused on is continuity of care for people with substance use disorder who are living with or at-risk of HIV during the COVID-19 pandemic.
