This report represents a landscape review of community-level efforts to address SDOH, followed by interviews with participants in three community-level initiatives that have built networks to coordinate clinical and social services. This report presents a cross-site analysis of the three initiatives, highlighting factors they identified as facilitating their efforts, the challenges they faced, their plans for continued expansion, and opportunities for federal and state entities, among other actors, to contribute.

This article parallels the works of the multidisciplinary academic group on children and coronavirus (MACC) in Israel with a multidisciplinary team from John Hopkins’ own efforts to create data-driven solutions to minimize health and education inequities for children throughout the COVID-19 pandemic. This project produced several outputs including a series of free, web-based, school-reopening modules for school and district administrators; a 2020-2021 School Reopening Tracker based on publicly available data sources; a Teacher & School Staff Vaccination COVID-19 Dashboard; and a COVID-19 Global Education Recovery Tracker. This dashboard provides an initial look into how to quantify the level of vulnerability within an area (as defined by ZIP code, ZCTA). Interacting with the data at this level allows local decision makers to understand the vulnerability of their populations, as well as the vulnerability of neighboring regions.

This case study describes the Colorado Department of Public Health and Environment’s (CDPHE) work to change the narrative and framing of population health data to emphasize the importance of structural inequities and social determinants of health. CDPHE is also working to incorporate equity metrics into program evaluations and performance monitoring and use boilerplate language regarding equity in CDPHE publications.

Data Collection Tool
The Neighborhood Atlas website was created in order to freely share measures of neighborhood disadvantage with the public, including educational institutions, health systems, not-for-profit organizations, and government agencies, in order to make these metrics available for use in research, program planning, and policy development. The Area Deprivation Index allows for rankings of neighborhoods by socioeconomic disadvantage in a region of interest (e.g., at the state or national level). It includes factors for the theoretical domains of income, education, employment, and housing quality.

This article provides a brief overview of a framework to incorporate data and community engagement into a targeted approach to promote equitable testing and treatment of COVID-19, and to address underlying social determinant of health risks that disproportionately burden racial minority communities.
