The study analyzed data from over 850,000 adult patients, noting that individuals with limited English proficiency or a preference for a non-English language experienced delays in receiving their first vaccine dose. These groups also had higher rates of hospitalization and death related to COVID-19. The Community Outreach Specialist (COS) program aimed to increase COVID-19 vaccine uptake among rural communities through targeted outreach efforts. The program trained and deployed community outreach specialists who engaged with community members, provided education about COVID-19 and vaccination, addressed concerns and misconceptions, and facilitated access to vaccination services. The COS program in reaching and engaging with socially vulnerable and medically underserved populations, successfully increasing vaccine acceptance and uptake in these communities by building trust, addressing barriers to vaccination, and tailoring outreach strategies to the local context. The article discusses the importance of community-based approaches, cultural competency, and collaboration with local organizations and leaders in achieving successful vaccine outreach. It also emphasizes the need for sustained efforts to ensure equitable access to vaccination and to address disparities in vaccine uptake among different populations.

Case Study
This article discusses the process used by the New York City Health Department to prioritize populations for access to the first round of COVID-19 vaccines. The city’s process was guided by input gathered during five public meetings. During these meetings, participants emphasized the need to prioritize health equity, examining the broad swatch of personal and societal factors that make people more or less susceptible to severe COVID-19 disease.

The Massachusetts Executive Office of Elder Affairs (EOEA) oversees programs and services for older adults through the aging service network, and worked closely with the COVID-19 command center during the COVID-19 pandemic. EOEA worked with senior affordable housing providers and advocated for vaccines for older adult populations. They also used their network to disseminate COVID-19 information to older adults, caregivers, and those working directly with older adults.

This study sought to assess the correlation between population and spatial characteristics of COVID-19 by examining positive COVID-19 data from New York City with ZIP Code Tabulation Areas to determine what social determinants of health increase an individual’s risk of contracting COVID-19. Results showed that neighborhoods with larger proportions of Black people, people older than 65, people with heart disease, and people living in densely populated housing are at higher risk of contracting COVID-19.

This study highlights the geographic differences in COVID-19 positivity rates within Los Angeles county, taking into consideration other socio-structural determinants that impact these rates. The data — testing data from the LAC Department of Public Health and American Community Survey data — was analyzed and mapped to include testing rates, race/ethnicity, poverty, insurance status, education, and population/household density. Results found higher COVID-19 positivity rates in communities with higher proportions of Latinx residents, those living below the federal poverty line, and those with high household densities.

This study looks at the association between county-level income inequality and COVID-19 cases and deaths from March 2020 to February 2021. This cohort study used county-level longitudinal data for all 50 states, Puerto Rico, and D.C. Results showed that income equality was associated with higher rates of COVID-19 cases and mortality. Targeting communities with higher rates of income inequality must be considered to lessen the burden of inequality.

In this project, RAND Corporation researchers identified existing health equity measurement approaches that might fit with Medicare’s VBP programs, quality reporting efforts, and confidential reports. The project had two objectives: identify health equity measurement approaches, and decide which of these approaches merit consideration for inclusion in Medicare’s VBP programs, quality reporting efforts, and confidential reports. This report describes the methods and findings of the project and delineates potential first steps for the U.S. Department of Health and Human Services to consider as it continues to evaluate the prospect of incorporating health equity measures and domains in Medicare’s VBP and reporting programs.
