To help policymakers and other stakeholders identify opportunities to improve health equity in their states, SHADAC has produced a set of data resources for the 50 states and the District of Columbia. Using the Behavioral Risk Factor Surveillance System (BRFSS) Survey — combining the three most recent years of data (2018–2020) to improve our ability to develop reliable state-level estimates for smaller population subgroups — SHADAC created maps and charts showing how states compare to the U.S. average in measures of people’s self-reported physical and mental health, and how people’s physical and mental health varies depending on their race and ethnicity, level of income, and age within each state.

This brief describes work to partner health departments with community organizing groups, focusing on a collaboration between Santa Barbara County Public Health Department and two community organizations. These partnerships allow health departments to actively address power imbalances that may decrease community trust in health departments. The Santa Barbara Public Health Department developed an analysis of power and worked directly with grassroots organizers. To build trust, the health department undertook many actions, including sharing organizational charts, explaining decisions, facilitating personal sharing and trust exercises, spending time together, discussing structural racism, and more. During COVID-19, these partners shared information targeted towards those with various languages and literacy levels.

In northwest Arkansas, the Marshallese community has experienced a disproportionate COVID-19 burden. This response plan, funded by the CARES act, hired English/Marshallese bilingual staff to provide contact tracing, navigation, and other types of response. Staff that were not bilingual participated in cultural humility training.

An early report issued by the CDC identified staff members working in multiple nursing homes as a likely source of spread of COVID-19. The authors performed the first large-scale analysis of nursing home connections via shared staff and contractors. Using a large-scale analysis of smartphone location data, they found that 49 percent of COVID-19 cases among nursing home residents was attributable to staff movement between facilities. Traditional federal regulatory metrics of nursing home quality were unimportant in predicting outbreaks. The results provide evidence for a policy recommendation of compensating nursing home workers to work at only one home and limit cross-traffic across homes.

The Massachusetts Department of Public Health (MDPH) initiated a quality improvement project to improve use of data for action to promote racial equity. To address the challenges identified in the study, the team identified potential solutions and prioritized development and piloting of the MDPH Racial Equity Data Road Map (Road Map). The Road Map framework provides strategies for data collection and uses that to support the direction of actionable data-driven resources to racial inequities. The Road Map is a resource to support programs to authentically engage communities; frame data in the broader contexts that impact health; and design solutions that address root causes.
