This case study of the VA Puget Sound Health Care System highlights effective practices for increasing vaccine uptake among vulnerable populations of veterans. This included targeting outreach to people based on race/ethnicity and the presence of risk factors for severe COVID-19 disease. The VA Puget Sound Health Care System used multiple modalities of communications and collaborated with community leaders in order to increase vaccine rates among people in underserved areas.

This article summarizes the lessons learned from implementing telehealth in rural environments over the past 3 years and how to expand these efforts in the context of COVID-19. The needs of elderly populations in rural environments are also addressed. This process overview summarizes the approach taken by the Alberta Health Services Design Lab to respond to the outbreak of COVID-19 among meat plant employees, who are largely immigrants. This article summarizes the approach one vaccine collaborative in Philadelphia, PA took to address vaccination inequity among Black community members. The intervention utilized community partners to build trust and encourage community members to schedule vaccinations at a clinic through a low-tech platform. These vaccination clinics were also designed to minimize wait times.

The case study outlines how Lake County Health Department (LCHD) in Illinois created a health equity team whose job included facilitating relationships with community and agency partners. The case study includes examples of how the team created tools to increase awareness within LCHD of the impact of social determinants on health. For the team, LCHD specifically prioritized candidates who had lived experience in historically vulnerable communities and those who were used to multicultural bridging.

This case study outlines how the Cuyahoga County Board of Health (CCBH) in Ohio helped to found, coordinate, and facilitate a multisectoral consortium that created a Community Health Improvement Plan for the county. The consortium also implemented trainings on equity. The CCBH’s participation in the consortium helped bring equity to the front of CCBH’s strategic plan.
