This report represents a landscape review of community-level efforts to address SDOH, followed by interviews with participants in three community-level initiatives that have built networks to coordinate clinical and social services. This report presents a cross-site analysis of the three initiatives, highlighting factors they identified as facilitating their efforts, the challenges they faced, their plans for continued expansion, and opportunities for federal and state entities, among other actors, to contribute.

This study uses data from the COVID Tracking Project’s Racial Data Tracker, which aggregates state-level COVID-19 reporting and tracking databases to determine racial/ethnic trends of COVID-19 incidence and evaluate the racial/ethnic distribution of COVID-19 related mortality in the US. Results found that disparities are more apparent at the county and city level, and discusses the importance of transparent, local data in order to allow for greater precision in resource allocation and effective policy changes aimed at reducing disparities. The study includes choropleth maps of the results by state.

The National Council of Asian Pacific Islander Physicians provides an analysis about the comprehensive collection and reporting of patient demographic data related to COVID-19 testing and treatment. The report makes six recommendations focused on reporting race and ethnicity data.

This report details the immense toll the COVID-19 pandemic has taken, and continues to take, on Black communities. The report’s authors – physicians and public health and policy experts – note massive disparities experienced by Black Americans. The worse outcomes, are a predictable result of structural and social realities. The predicates that caused these disparities have long been present. These are cycles we have to break. The report highlights areas for immediate focus and attention, and presents a set of expert-generated recommendations for action.

This case study describes the Colorado Department of Public Health and Environment’s (CDPHE) work to change the narrative and framing of population health data to emphasize the importance of structural inequities and social determinants of health. CDPHE is also working to incorporate equity metrics into program evaluations and performance monitoring and use boilerplate language regarding equity in CDPHE publications.
