White Paper/Brief
The Centers for Medicare & Medicaid Services is encouraging higher vaccination rates at nursing homes. Vaccinations are required to be offered at nursing homes, yet the overall vaccination rate is below 50%. The article provides recommendations for ways nursing homes can increase vaccinations by education, communication with residents and their families, and creating action plans for individual residents. It stresses the importance of keeping treatment medications at nursing facilities.

Case Study
This paper describes how a local public health agency joined a multi-sector community partnership made up of community-based organizations, clinical providers, and peer navigators and others with lived experience of homelessness. Together, this group created a strategic plan to lessen the impact of COVID-19 on people experiencing homelessness. They prioritized education, increased resource allocation and testing in shelters, and slated housing for those who tested positive or those at increased risk of COVID complications. Most of this work took place in shelters.

Case Study
This article describes how one county’s health department partnered with emergency services to form a strike team focused on mitigating COVID-19 outbreaks in congregate living facilities. The team helped create more cohesive communication between these facilities and between facilities and the health department. This strike team identified these facilities through partnerships with community organizations. The strike team conducted wellness checks, provided PPE education, and helped facilities secure PPE. In facilities with potential outbreaks, the team provided early testing and quick isolation.

Case Study
This report collects the strategies that urban health departments used to protect residents experiencing homelessness or at risk of experiencing homelessness during COVID-19. Strategies included working with community partners (particularly shelters) to expand housing, increasing outreach and resource allocation to people experiencing homelessness, and building better communication networks.

Implementation Guide
This guidebook was designed to increase equity and access in COVID-19 testing and contact tracing in Virginia. The guidelines included a health equity framework, recommendations for working with communities, common barriers to COVID-19 testing faced by the most at-risk populations, and how to identify areas with unmet testing needs. Descriptions of how to determine appropriate interventions for testing, contact tracing, and building trusting community relationships are included.

Summary Report/Recommendations
The resource presents best practices for increasing access to COVID-19 testing and vaccination sites for people with disabilities or special health needs. It highlights physical, sensory, cognitive, and technological ease of access, and gives practical ideas for improving access throughout all steps of the testing/vaccination experience. Ideas include topics from accessible parking to ensuring clear communication despite the use of face masks.

Case Study
This article lists key findings and considerations for jurisdictions and communities regarding ongoing COVID-19 pandemic operations across the country as they relate to persons with disabilities. For example, individuals may need additional support to shop online for food or connect with organizations for food access. In addition, access to COVID-19 information may be difficult to access if websites are not 508 compliant, and a lack of internet access can compound challenges. Finally, some case studies are included for how communities responded to these challenges.

Peer Review Study
This case study is on the effectiveness of COVID-19 vaccine distribution via mobile vans to residents/staff of 47,907 long-term care facilities (LTCFs) across the United States that relied on algorithms to optimize vaccine distribution. The authors developed a modeling framework for vaccine distribution to high-risk populations in a supply-constrained environment. The framework decomposed this challenge as two separate problems: an assignment problem, where they optimally mapped each LTCF to select CVS stores responsible for vaccines; and a scheduling problem, where they developed an algorithm to assign available resources efficiently. The learning and this framework may be of use to other organizations, including communities where mobile clinics can be established to efficiently distribute vaccines and other healthcare resources in a variety of scenarios.
