White Paper/Brief
An early report issued by the CDC identified staff members working in multiple nursing homes as a likely source of spread of COVID-19. The authors performed the first large-scale analysis of nursing home connections via shared staff and contractors. Using a large-scale analysis of smartphone location data, they found that 49 percent of COVID-19 cases among nursing home residents was attributable to staff movement between facilities. Traditional federal regulatory metrics of nursing home quality were unimportant in predicting outbreaks. The results provide evidence for a policy recommendation of compensating nursing home workers to work at only one home and limit cross-traffic across homes.

Systematic Review/Meta-Analysis
This review summarizes the factors contributing to COVID-19 inequities among immigrant communities. The review concludes with the overarching recommendation of engaging and empowering immigrant communities. Facilitating factors to address health inequities in this population include providing linguistically appropriate services, addressing poverty, and ensuring undocumented immigrants have access to public benefits.

Systematic Review/Meta-Analysis
A review of best practices for COVID-19 infection prevention and control in long-term care facilities. These included establishing surveillance measures, revising staffing and visitor policies, and clearly communicating health measures and case numbers. The authors highlight the need for additional support and resources for long-term care facilities to address the pandemic over time. The article also provides updated guidelines for rapid situation analyses.

Case Study
This article describes testing efforts in Georgia’s long-term care facilities and the impact that preventative routine screening had on COVID-19 cases among residents/staff. Facilities that had regular preventative testing rather than testing only after learning of a confirmed case had a lower initial prevalence of COVID-19 and fewer subsequent cases.

Summary Report/Recommendations
This practice describes a Three-Phase Approach to mitigating COVID-19 in long-term and post-acute care nursing facilities in the Seattle, WA area. The authors outline a structure for addressing the pandemic based on disease surveillance measures, with different focus areas within each phase. Measures include:
(1) Initial: Communication, tracking, PPE preparation
(2) Delayed: Education, testing, isolation
(3) Surge: Activation of a “drop team”” of health care professionals during an outbreak

Peer Review Study
This case study is on the effectiveness of COVID-19 vaccine distribution via mobile vans to residents/staff of 47,907 long-term care facilities (LTCFs) across the United States that relied on algorithms to optimize vaccine distribution. The authors developed a modeling framework for vaccine distribution to high-risk populations in a supply-constrained environment. The framework decomposed this challenge as two separate problems: an assignment problem, where they optimally mapped each LTCF to select CVS stores responsible for vaccines; and a scheduling problem, where they developed an algorithm to assign available resources efficiently. The learning and this framework may be of use to other organizations, including communities where mobile clinics can be established to efficiently distribute vaccines and other healthcare resources in a variety of scenarios.

This article describes the approach and impact of the Stanford Flu Crew, a service learning program at Stanford University School of Medicine, where pre-clinical students provide vaccines to underserved populations in community settings. The article includes information on both program outcomes (i.e., the number of people vaccinated per year over a 4-year period) and student perceptions of learning outcomes achieved through this program.

Case Study
This case study outlines how the Cook County Department of Public Health (CCDPH) in Illinois partnered with community organizers on worker and immigrant rights to create a multisectoral collaborative to address root causes of health inequity called the Collaborative for Health Equity. This collaborative explicitly works to address structural racism and health inequities. It has built relationships with local social justice organizations that have helped guide CCDPH to center health equity in its strategic plans.

Peer Review Study
This article reviews systemic factors affecting low income immigrant communities during COVID-19 and provides recommendations strategies to improve public health infrastructure using the Public Health 3.0 concept. This high-risk community represented a large portion of essential workers, who even before the pandemic faced less access to health care and were structurally marginalized. The authors discuss equity strategies including prioritizing COVID-19 public relief funds and allocating testing and vaccines to these communities. The paper also reviews how to improve public health infrastructure to mitigate disparities immigrant communities face by addressing social determinants of health, communicating about and structuring policies and programs that do not require identification, engaging cross-sector stakeholders, and developing tools to collect relevant, appropriate data.
